Map disappeared

I'm trialling putting google maps onto my websites, I went live earlier and a couple of the sites are still working, but the main one has gone blank. There's no map at all, just a blank area. Erm .. it was working fine earlier and the other sites use the same base code.

Any reports of users getting banned for usage? How do they know? I'm pretty sure the usage is within the T&C's.
