The easiest way to populate a form is to put an "id" attribute on each
of the <input> elements, then you can write
        document.getElementById("lon_form").value = LongPoint;
         document.getElementById("lat_form").value = LatPoint;

Like this http://www.econym.demon.co.uk/googlemaps/opp/input.htm

Other things to note on your page:

  You only need to load the API code once

  Your latitude and longitude were the wrong way round:
  lat is y, long is x
